Consumer Directed Care Plus (CDC+) Background Screening Requirement Notification

All CDC+ Representatives, with the exception of self-representatives, are required to undergo a background screening, pursuant to 409.221 and s.393.0655, Florida Statutes (F.S.).

If you have not completed the requirements, you must submit the required coversheet and results to apd.cdc.backgroundscreenings@apdcares.org by no later than December 31, 2025, to avoid the removal of the CDC+ Representative and the Consumer's disenrollment from the program due to non-compliance.
